- Work email (required)
- Office email and secondary email (if applicable)
- Notes (e.g., coordinator roles, assistant contacts)
Who should complete this?
We recommend that your office administrator, HR, Chief of Staff, or another key point person fill out the template to ensure accuracy and completeness.
